When you're intubating a patient, twiddling your thumbs waiting for your induction and paralytic to work, do you ever ask yourself "hmm, should I be bagging this patient?"

Hi Herb, in terms of BVM peri-intubation, I think the two questions that come into play there are "is my patient at high risk for desaturation?" and "are they at high risk for aspiration?". If the first answer is no I will keep flush rate NC oxygen on during the apneic period and not bag. If yes to question 1, on to question 2. If not high risk for aspiration then gentle BVM during the apneic period (45-60 sec) is fine. If they are high risk for aspiration I would consider ketamine based DSI (delayed sequence intubation) so they continue to respire during the induction phase.

In terms of BVM, do you have a length of time that you would do this, or do measures such as SaO2 andcapmography control this? Thanks Herb D RN
